Eligibility

Am I eligible for coverage?

Benefit-eligible team members (regular team members with standard hours in Workday of 24 hours or more) are eligible for medical benefits the first of the month following date of hire and can make plan changes during open enrollment or with a qualifying life event.

Who can I cover?

Eligible dependents are your spouse, qualified domestic partner, children and domestic partner children, up to age 26 or if disabled.

Plan Options, Coverage and Costs

Sleep Number offers three medical plans. All plans have an out-of-pocket maximum. If you happen to reach the out-of-pocket maximum, the plan covers your remaining eligible expenses at 100%. All plans have coverage for transgender services and assisted fertilization.

This plan has a high deductible. After reaching the deductible, you pay coinsurance.

This plan also comes with a Health Savings Account (HSA) – a pre-tax account that you and Sleep Number can contribute to.

  • In 2023, Sleep Number will contribute $19.23 per pay period ($38.46 per pay period for team members + dependents) into your HSA. For a full year, this is $500 or $1000, respectively. To receive this contribution, you must enroll in the HDHP and HSA plan in Workday.
  • If you have other coverage, you may be disqualified from having an HSA. Contact HR for more details.

You also have access to condition support programs such as Omada for diabetes prevention/management.
